Thank you, this article needed to be written, and I hope it's widely read. Another phenomenon that happens, usually in "fine dining" settings, is to loudly send food back to the kitchen. While sometimes necessary (perhaps your meat is too rare), this can be handled quietly--but these people will loudly exclaim "this is inexcusable!" or "take this away, I can't even look at it!" (They will often then use this a reason for not tipping.) They believe themselves to be showing other patrons that they are accustomed to an even higher standard of service, but simply mortify everyone within earshot. Dining doesn't have to be drama!
