Crystal: A family member did this for me a long time ago. She collected my birth day and month and year & place of birth - and for the moon sign, especially needed my time at birth - the closer you are to the exact time, the more it's supposed to be accurate.

It was interesting because I'm on the cusp and always thought I was a Gemini, but

she had it calculated and it turned out I am a Taurus.

I am still working on updating things.  I hope to have a moon chart up soon.  There are some books I have listed there that have moon charts right in them.  You need your birth date, the location where you were born, the time you were born, and then that can be looked up or calculated. 

Linda Goodman's Relationship signs is a fabulous book with so much information in it, including an Ephimaris, where you would look up your Moon sign.  Your rising sign is of equal importance, also.  I will link you to that book.  Its cheap and mine is so tattered and worn from use that I may need to buy one too lol! It is also available used, which I would suggest.
